Online Patient Registration
To make your visit as convenient as possible, we offer online patient registration. No more filling out endless pieces of paper! Instead, 3 days before your scheduled appointment (or sooner if you request) a link will be texted and/or emailed to you. From that link you can access the forms you need to complete. It is mostly check boxes to click and should take you less than 15 minutes to complete. Plus, you’ll now be able to look up your balance and make payments or set up a payment plan all from your account. Learn more about our pre-registration system.
